  If you are a teacher and want to have access to survey's, we must validate your identity as a professor. To do this, please fill out the form with your full name, and your university email address. No other email addresses will be accepted. Also, you must provide your desired username and password to enter the ChemX teacher panel. Please note, for added security, do not use the same password as your email or another other widely accessed accounts.

Please use the name that appears when students register for their university classes so they can recognize it when using the survey. Please note that if you teach more than one class, you should denote this as shown above by including commas (,) between each class taught.
